You may visit the Mesirow Insurance Services, Inc. web site to create and print a required proof of the association's insurance. Follow the steps below to create and print a certificate.
The certificate will display in a "pop-up" window, so you must allow pop-ups to view and print the certificate.
1. Click Here Log on to the Mesirow's Certificate Request Website
3. Use the Login E-Mail Address
4. Use the Password sandburg
5. Select "I Accept" and then Click "Login"
6. Type “Carl Sandburg Village” into the search box
7. Select “Carl Sandburg Village Condominium Association #2 (1455 & 1460 N. Sandburg Terrace, Chicago IL)”from the list of results that appears
8. Click on "Next"
9. The Association's Information will be filled in for you. Enter your personal information in the blank spaces under Unit Owner Information, and your mortgage company’s information in the blank spaces under Certificate Holder / Mortgage Clause
10. Click on "Submit" and your certificate will be created
The certificate will be displayed in a new window. You may save or print the certificate. |
